Requirements
- Complete
the General Information Form (in the Aveda Institute Chicago catalog)
and submit it to the Admissions Office.
- Have
a career planning session with an Admissions Representative. To schedule
a session, complete the form above or call the Admissions Department
at 773.883.1560 x402.
- Submit
a signed acknowledgement that you have received a student information
folder which contains: licensing requirements, physical demands, safety
requirements and job demand survey.
- Take
and pass the entrance exam.
- Submit
a typed letter of intent describing your reasons and goals for enrolling
in the program
- Submit
a letter of recommendation from an employer, counselor, teacher, or
the leader of a volunteer or special interest group which you are
affiliated. Letters from family members or personal friends will not
be accepted.
- Submit
an official copy of your high school transcripts
- Submit
an official copy of your high school diploma or General Education
Diploma (GED).
- Submit
a photocopy of a picture ID, i.e. Driver's License.
- Submit
a completed immunization form in accordance with Illinois State Law.
- Complete
a Release of Information/Emergency Contact Form.
- Submit
a signed copy of the Privileged Information Form.
- Submit
a completed enrollment contract (to be provided by your Admissions
Representative) with a non-refundable $100 application fee.
The
admission requirements are the same for both new students and students
who are re-enrolling in the Aveda Institute Chicago. The Aveda Institute,
in its admissions, instruction and graduation policies, practices no
discrimination on the basis of race, religion, color, financial status,
sex, national origin, age, veteran or sexual orientation. The Aveda
Institute does not recruit students that are currently attending or
admitted to another school offering a similar program of study.
